Plant-based diets and blood pressure

Plant-based eating lowers blood pressure by a few millimetres of mercury, small for one person but the kind of shift that moves population risk.

The short version

The strongest single read on plant-based eating and blood pressure is a 2014 JAMA Internal Medicine meta-analysis that pooled 7 controlled trials and 32 observational studies. Because it includes randomized trials, it can speak to cause rather than correlation alone, and in those trials the drop in systolic pressure was small but clear.

The observational studies, which watch people eating as they normally do rather than assigning a diet, showed a larger gap. Observational numbers run bigger because the people who already eat this way differ in other ways too.

A large cohort agrees

Adventist Health Study-2 checked the same question in a 500-person clinical sub-study and found vegetarians, and vegans especially, sitting lower, while being honest that some of the effect traces to body weight rather than diet directly.

How big is a few mm Hg?

For one person, a 5 mm Hg drop is modest. Across a population it is not: shifts of that size move the rates of stroke and heart disease measurably, which is why the effect matters even though the per-person number looks small. The Academy of Nutrition and Dietetics puts the observational gap in the same range.

The grade here is solid. Randomized trials back a real causal effect, cohorts and a large meta-analysis agree on the direction, and the honest caveat is only about size and mechanism: the drop is modest and partly mediated by weight, not a substitute for medication where it is needed.
